Absence Request Policies

1) You must submit your request at least one (1) week before the potential absence.

2) Just because you submitted a request does not mean that your absence is excused. Mr. Gonzalez must still approve of your request. 

 

3) This form is meant to be a LAST RESORT. Please attempt to schedule all appointments or other needs on days that do not conflict with you commitment to the music program.
